When the thrust reverser on Lauda Air Flight 004 accidentally deployed in mid-air, it created extreme asymmetrical drag. The engine essentially blasted hurricane-force winds against the aircraft's forward momentum, shattering the airflow over the left wing. Without lift, the massive Boeing 767 went into an irrecoverable roll, breaking apart under supersonic G-forces.
Electronic safeguards failed that day. In response, the FAA mandated the installation of physical, mechanical "sync locks" on all thrust reversers. These heavy-duty titanium bolts physically lock the engine cowlings during flight, making a mid-air deployment mathematically impossible.
